Elevated Care: Striving for Excellence

Let me set the scene, as a pediatric speech language pathologist, with a passion for feeding therapy, I've experienced firsthand how collaboration can transform treatment outcomes.

I once worked with a complex feeding patient who had challenges participating in therapy in order to safely chew and swallow food. As a newly graduated SLP, I had no clue who to go to for help. Luckily I was working in a highly collaborative multidisciplinary clinic and it dawned on me that I needed support from a physical therapist.

Once we figured out that the child lacked the core strength to sit at the table for extended periods, his progress towards safe eating accelerated dramatically. Without proper postural support, his developing brain was preoccupied with maintaining balance, leaving little room for learning to chew and swallow.

This is just one example of why transitioning from multidisciplinary to transdisciplinary care is so critical in pediatrics. By integrating expertise across disciplines, we can create more effective and sustainable interventions that support the whole child.

Supporting neurodivergent children requires a thoughtful and collaborative approach that integrates expertise across disciplines. By elevating care through multidisciplinary, interdisciplinary, and transdisciplinary models, professionals can better address the diverse needs of children using a whole-child approach. Here's how these models differ and why collaboration among Applied Behavior Analysis (ABA), Occupational Therapy (OT), Physical Therapy (PT), Speech Therapy (ST), and Mental Health professionals is essential.

Understanding the Models

  1. Multidisciplinary Care:

    • Professionals work independently within their areas of expertise while sharing information about the child’s progress. For example, an OT may focus on sensory processing while an ST addresses communication challenges, without blending their methods.

    • Strength: Clear delineation of roles ensures specialized care.

    • Limitation: Limited collaboration may miss opportunities for holistic solutions.

  2. Interdisciplinary Care:

    • Experts maintain their scope of practice but collaborate more closely to align goals and strategies. For instance, a PT working on mobility might coordinate with an OT focusing on fine motor skills during daily activities.

    • Strength: Enhanced communication fosters integrated care.

    • Limitation: Collaboration may still be constrained by professional boundaries.

  3. Transdisciplinary Care:

    • Professionals transcend traditional boundaries, sharing knowledge and skills to deliver unified interventions. For example, an ABA therapist might incorporate sensory strategies from OT into behavior plans.

    • Strength: Holistic, child-centered care that adapts to overlapping needs.

    • Limitation: Requires extensive training and trust among team members.

The Importance of Collaboration

Neurodivergent children often face challenges across multiple domains—regulation, sensory processing, communication, motor skills, and emotional health. Collaboration among ABA, OT, PT, ST, and mental health professionals ensures these needs are addressed comprehensively:

  • Behavioral and Sensory Integration: ABA can reinforce positive responses to sensory inputs addressed by OT.

  • Communication and Motor Skills: ST strategies can be integrated into PT exercises to enhance both language development and physical coordination.

  • Emotional Well-being: Mental health professionals can provide trauma-informed care while collaborating with other therapists to support emotional regulation.

Elevating the Whole-Child Approach

A whole-child approach recognizes the interconnected nature of development in neurodivergent kids. It emphasizes:

  • Holistic Assessments: Evaluating strengths and needs across domains like movement, communication, and behavior.

  • Unified Goals: Setting shared objectives that reflect the child’s unique profile.

  • Consistency Across Settings: Reinforcing strategies at home, school, and therapy sessions for seamless support.

By elevating collaboration across disciplines, we can create an inclusive environment that empowers neurodivergent children to thrive in all aspects of life.
